Swimming South Africa and Swim CATS Swim Club

The following information regarding the registration for the 2010/2011 is very important, please read it very carefully.

Each person registering as a competitive swimmer or official must complete the registration forms listed below in full.

Please ensure that the CORRECT FIRST NAME, as it appears on the Birth Certificate/ID Document is filled in [e.g. Robert and not Bob] (There is a space for a preferred name to be filled in).

If this is a first time registration, a copy of the Birth Certificate/ID document must be attached to the capitation form. A COPY OF A DRIVER’S LICENCE WILL NOT BE ACCEPTED.

PLEASE ENSURE THAT ALL THE INFORMATION ON THE FORM IS PRINTED CLEARLY. PLEASE FILL IN ALL THE DETAILS ON THE FORM AND MAKE SURE THAT THE FORM IS SIGNED BY THE SWIMMER AS WELL AS THE PARENT/GUARDIAN IF THE SWIMMER IS UNDER THE AGE OF 18. NO FAXED FORMS WILL BE ACCEPTED.

Please note that no swimmer will be allowed to participate at any gala unless he or she is a registered member of Swim South Africa.
